Care Management ON-Call was designed for those who are aging in place and are "doing okay" for now.  But you and your families are concerned about that telephone call in the middle of the night - your health status has changed, you're on your way to the Emergency Room.

For a modest flat fee, we will do a brief assessment on you and your spouse and gather some basic, but necessary, documentation:  your Powers of Attorney, Living Will, Funeral Plan, Insurance Cards, and your Medical Records.  By obtaining these documents, and interviewing you and your family to determine your unique situational needs, we will be ready to step in on a moment's notice.

We will then be ON-CALL for you for one year following the assessment and will contact you on a monthly basis to determine if anything regarding your situation has changed substantially.

If an emergency occurs, you or your family representative can contact us 24 hours a day, 365 days per year, to step in and handle any crisis that may occur for an hourly fee.  We can act as a liaison between your family and your situation - healthcare providers, facilities, physicians, etc.  We can ensure that your wishes are adhered to as specified in your Advanced Directives, that your family is kept apprised of your condition

We can allay your family's, and your, fears, until the situation is under control.
